Qualifications

• Graduated from a CAPTE-accredited Physical Therapy program or equivalent.
• Passed the state license examination for physical therapy.

Responsibilities

• Provide skilled physical therapy in home setting per physician plan.
• Assist evaluation and revise plan of care as needed.
• Treat to relieve pain and restore function using PT modalities.
• Document patient status and caregiver education for discharge.
• Educate patient/caregiver and healthcare team on PT care.
• Promote coordination of care through team conferences.
