Qualifications

• Current Physical Therapist license in the state of practice.
• Graduate of CAPTE-accredited Physical Therapy program.
• Passed the state licensing examination.
• Meets federal and state qualification requirements for Physical Therapists.

Responsibilities

• Provide skilled physical therapy according to the physician's plan of care.
• Evaluate patients using appropriate assessment tools and develop/update individualized care plans.
• Deliver treatments to reduce pain, restore function, improve mobility, and maximize independence.
• Monitor, document, and report patient progress to physicians and managers.
• Educate patients, caregivers, and healthcare team on therapy techniques and home exercises.
• Reassess patient needs and revise treatment plans as necessary.
• Initiate diagnostic, preventive, and rehabilitative interventions for safety and recovery.
• Coordinate care with physicians and interdisciplinary team through meetings.
• Maintain accurate, timely clinical documentation.
• Participate in quality improvement, utilization reviews, staff meetings, continuing education.
• Stay current with PT best practices and regulatory standards.
• Provide staff education as directed.
• Supervise Physical Therapy Aides at least every 30 days when applicable.
