Qualifications

• Current license to practice physical therapy in the relevant state.
• Graduation from an CAPTE-accredited PT program.
• One year of PT experience in home health preferred; mentoring may be required for less experience.
• Current CPR certification.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations (every 30 days at minimum), modify interventions as needed and perform discharge planning.
• Other duties as assigned.
