Job summary

Neogen Care in Honolulu, HI is seeking a Physical Therapist to provide home health services to patients in the Honolulu area. The role requires delivering skilled PT, developing and adjusting plans of care, and collaborating with physicians and care teams to achieve functional goals.

Candidates must hold a Doctor of Physical Therapy (DPT), an active Hawaii license, CPR/BLS certification, and at least one year of home health or institutional PT experience.

Qualifications

• Doctor of Physical Therapy (DPT) from an APTA-accredited program.
• Active Hawaii physical therapy license.
• Current CPR/BLS certification.
• Minimum 1 year of PT experience in home health or institutional setting.
• Valid driver’s license with a reliable vehicle.

Responsibilities

• Plan and administer prescribed PT treatment to achieve maximal function.
• Screen patients and document baseline and progress.
• Develop and adjust plans of care in coordination with physicians.
• Communicate with clinicians to update treatment plans and goals.
• Supervise staff and provide clear care plans for assistants.
