Job summary

Neogen Care seeks a Physical Therapist for Home Health in Honolulu, HI. You will plan and deliver skilled PT to patients in their homes, collaborating with the physician and clinical team to achieve functional goals.

Requires a DPT, Hawaii license, and CPR/BLS certification, plus 1+ year home health and institutional PT experience. Valid driver’s license and reliable vehicle are required. Excellent communication and documentation skills are essential.

Qualifications

• DPT degree from an APTA-accredited program.
• Current Hawaii PT license.
• Current CPR/BLS certification.
• At least 1 year PT experience in institutional settings.
• At least 1 year home health PT experience.
• Valid driver’s license with reliable vehicle.
• Strong observation, communication and problem-solving skills.

Responsibilities

• Plan and administer prescribed physical therapy for home health patients.
• Assess patient function and set measurable treatment objectives.
• Develop and adjust rehab plans with physicians and the care team.
• Supervise therapy assistants and home health aides.
• Document progress and care notes accurately and promptly.
• Communicate progress to physicians, managers, patients and families.
• Maintain compliance with state regulations and professional standards.
