Job summary

Neogen Care is seeking a part-time Physical Therapist for our Hawaii home health agency in Kapolei. The PT will perform skilled evaluations, develop treatment plans, and coordinate with physicians and a care team to meet patient goals.

Required are a DPT, Hawaii PT license, CPR/BLS, and at least 1 year of recent PT experience in home health or institutional settings. Driving to Kapolei area locations is expected.

Qualifications

• Must hold a Doctor of Physical Therapy (DPT) from an accredited program.
• Current Hawaii Physical Therapist license.
• Current CPR/BLS certification.
• 1 year of recent PT experience in institutional settings.
• 1 year of PT experience in home health.
• Valid driver's license with reliable vehicle.

Responsibilities

• Plan and administer skilled PT treatment to patients to improve function.
• Screen patients and record functional status to guide treatment.
• Develop and implement rehab plans and adjust as needed.
• Collaborate with physicians and care team on plans.
• Provide guidance to PT assistants and home health aides.
• Document care observations, plans and discharge summaries.
• Communicate patient progress to physicians and care team.
