Aspire Talent Solutions is seeking a Physical Therapist to join our home health team serving patients in and around Denver/Lancaster. This full-time role focuses on delivering compassionate, skilled care in patients' homes and documenting progress per physician orders.

The ideal candidate has at least nine months of PT experience, CAPTE-accredited degree, state licensure, CPR certification, and willingness to travel to patient homes.

Formación

• Minimum nine months experience as a Physical Therapist.
• CAPTE-accredited PT program.
• Currently licensed to practice as a PT.
• Must possess and maintain CPR certification.

Responsabilidades

• Provides patient care and documents treatment per physician orders and agency policy.
• Performs patient evaluations per agency policy and procedure; selects appropriate diagnostic material.
• Informs the physician of assessment results and recommended interventions prior to initiating care.
• Develops and documents treatment plans with chosen interventions and priorities.
• Provides organized treatment including advising, educating and counseling the patient.
• Presents pertinent patient information to physicians and keeps the team informed of status and plan.
• Delivers treatment within agency and payer guidelines according to patient needs.
• Determines measurable goals in collaboration with team and patients.
• Meets productivity standards and uses work hours efficiently.

Are you a dedicated Physical Therapist looking to make a real difference in the comfort of patients' homes? We're seeking a compassionate and skilled PT to join our home health team in the vibrant city of Lancaster. This full-time position offers the unique opportunity to provide personalized care to our diverse client base, helping them achieve their health and mobility goals.

What You’ll Do:

• Provides patient care and documents treatment in accordance with physician orders, agency policy and patient needs.
• Performs patient evaluations in accordance with agency policy and procedure. Selects appropriate diagnostic material to assess all aspects of patient care.
• Informs the physician of the results of the assessment and recommended clinical interventions prior to initiating care.
• Develops and documents appropriate treatment plans for patients including selecting appropriate intervention approaches, strategies, materials and establishing treatment priorities.
• Provides an organized and efficient patient treatment which includes advising, educating and counseling the patient.
• Presents pertinent patient information to physicians and keeps team members informed of patient status and treatment plan.
• Provides appropriate treatment to patients according to patient needs and within the parameters set by the agency and payer.
• Determines appropriate goals, which are objective and measurable in nature, in collaboration with team members and patients.
• Meets agency productivity standards and uses work hours efficiently.

What We’re Looking For:

• Minimum nine months experience as a physical therapist
• Preferred community/home health experience

Required Certifications and Licensures:

• • Master’s degree in physical therapy approved by CAPTE accredited physical therapist education program
  • Currently licensed to practice as a physical therapist within the state of agency operation and compliant with accepted professional standards and practices
  • Must possess and maintain valid CPR certification while employed in a clinical role
  • Must be a licensed driver who can travel to all business locations
