Job summary

Enhabit Home Health & Hospice seeks a licensed Physical Therapist to join our team providing care to patients in home settings. You will examine and treat mobility impairments using therapeutic modalities to improve function and independence, following physician orders and the established plan of care under the Branch Director's supervision.

Applicants must hold a valid state license and a reliable driver's license with dependable transportation.

Qualifications

• Must possess a valid state driver's license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Examine and treat patients with physical impairments using physical modalities.
• Help patients improve mobility and self-care in daily living.
• Administer skilled care per physician orders and plan of care.
• Teach family and other health-care team members as needed.
