Job summary

Optum's UP Home Health Portage is seeking a Physical Therapist to perform assessments, determine treatment plans, and deliver interventions in patients' homes. The role emphasizes improving mobility and function with thorough documentation and coordination with care teams.

Responsibilities include evaluating functional status, developing plans, reporting progress, and educating patients and families. Licensure and a valid driver's license are required; CPR is preferred.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Driver's license with vehicle insurance or access to dependable transportation.
• CPR certification (preferred).

Responsibilities

• Evaluates the patient's functional status and PT needs; assists in development of the plan of care.
• Provides PT services within the scope of practice and plan of care.
• Observes and reports patient response to treatment to supervising staff.
• Treats patients to relieve pain and restore function.
• Instructs patient and family regarding plan of care, limitations and progress.
• Completes progress notes after visits and submits per policy.
