Job summary

UnitedHealth Group's Atrinity Home Health is seeking a dedicated Physical Therapist to perform assessments, develop treatment plans, and deliver therapeutic interventions in patients' homes under the LHC Group umbrella.

You will evaluate functional status, coordinate with the care team, and document progress to support optimal outcomes for diverse populations. Competitive annual pay with benefits is offered.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Reliable transportation to patient homes.
• Ability to serve patients of diverse backgrounds.

Responsibilities

• Evaluates the patient's functional status and PT needs.
• Provides PT services within scope and plan of care.
• Observes and reports patient response to treatment.
• Treats patients to relieve pain and restore function.
• Instructs patients and families about plan of care.
• Visits patients per Plan of Treatment and submits progress notes.
• Completes PT evaluations and develops plan of care.
• Reports outcomes of evaluation and projected frequency of care.
