Personal Touch Home Care is seeking a licensed Physical Therapist to provide direct PT services in patients' homes in Columbia, KY. You will evaluate functional status and contribute to care plans tailored to individual needs.

Formación

• Licensed to practice a physical therapist in the State (KY).
• Degree from a baccalaureate or master's program in physical therapy approved by APTA or AMA bodies.
• Prefer one year of home health experience.
• Demonstrates good verbal and written communication and organization skills.

Responsabilidades

• Assess the therapeutic, rehabilitative, and functional status of patients.
• Participate in developing comprehensive care plans tailored to each patient.
• Evaluate home environments to support rehabilitation goals.
• Assess muscle strength, mobility, gait, ROM, and rehabilitation potential.
• Instruct patients/caregivers in use and care of therapeutic appliances.
