Lifeline Homecare, part of LHC Group and within the Optum family, seeks a Physical Therapist for home health in Kentucky. You will perform assessments, develop and implement a plan of care, and coordinate with the care team to improve patient function and comfort.

Requirements include active Physical Therapy licensure in Kentucky and a valid driver's license with reliable transportation. CPR certification is preferred.

Qualifikationen

• Current Physical Therapy licensure in state of practice (KY).
• Current driver's license and access to a dependable vehicle or public transportation.
• CPR certification preferred or to be obtained within 90 days of hire.

Aufgaben

• Evaluates the patient's functional status and PT needs.
• Develops and updates the plan of care with the physician.
• Delivers therapy to relieve pain and restore function.
• Coordinates with the health care team and educates patient/family.
• Documents progress notes after each visit and tracks outcomes.
• Complies with state practice guidelines and plan of care.
• Assesses response to treatment and adjusts as needed.
• Visits patients according to Plan of Treatment; reports progress to supervisor.
