Job summary
Personal Touch Home Care of Kentucky Inc is seeking a licensed Physical Therapist to provide in-home PT services in Somerset, KY, under physician plans of care. You will assess patients, develop and adjust care plans, and evaluate home environments to support rehabilitation goals.
Applicants should hold an approved PT degree, be licensed in Kentucky, and preferably have 1 year of home health experience. We offer a comprehensive benefits package and a supportive team culture.
Qualifications
- Licensed to practice a physical therapist within the State.
- Degree from a baccalaureate or master's program in physical therapy approved by APTA/AMA.
- Prefer one year of experience; home health experience preferred.
- Demonstrates good verbal and written communication and organization skills.
Responsibilities
- Assess patients' therapeutic, rehabilitative, and functional status.
- Participate in developing comprehensive care plans tailored to each patient.
- Evaluate home environment to support rehabilitation goals.
- Assess muscle strength, mobility, gait, range of motion, and rehabilitation potential.
- Instruct patients/caregivers in the use and care of therapeutic appliances.
