Job summary

Enhabit Home Health & Hospice is seeking a licensed Occupational Therapist (OT) to deliver in-home therapy focused on improving daily living activities and functional mobility for patients. This role requires applying skilled OT services per physician orders and the established plan of care, with oversight by the Branch Director.

The candidate will educate family members and care teams, coordinate care across disciplines, and contribute to high-quality, compassionate home-based patient outcomes.

Qualifications

• Graduate of an approved school of Occupational Therapy.
• Must be licensed as an OT in the practicing state.
• Medicare home health experience is preferred.

Responsibilities

• Help patients in the home to improve daily living skills and motor function.
• Provide skilled OT services per physician orders and plan of care.
• Educate family and health care team and coordinate with Branch Director.
