Job summary

St. Luke's University Health Network is seeking an Occupational Therapist for Home Health. You will work with an interdisciplinary team to deliver OT services in patients' homes, following physician orders and agency policies.

The role requires a Bachelor's degree in Occupational Therapy, licensure in PA/NJ, CPR certification, and 1–2 years of related experience. A Master’s degree is preferred. You will evaluate, treat, discharge planning, and educate patients and families.

Qualifications

• Bachelor's degree in Occupational Therapy required.
• CPR certification required.
• 1–2 years of experience preferred; Masters degree is a plus.

Responsibilities

• Performs initial evaluation to identify deficits and establish treatment goals.
• Deliver OT services in patients' homes and educate patients and families.
• Plan discharge and coordinate with Rehab Program Manager and staff.
• Maintain timely, accurate documentation per agency policy.
