Job summary

St. Luke’s University Health Network seeks an Occupational Therapist Home Health to provide quality therapy services in patients' homes. This role includes evaluating patients, establishing treatment plans in consultation with physicians, and guiding patients in therapeutic activities.

The ideal candidate will hold a Bachelor’s Degree in Occupational Therapy and possess a valid OT license in their state of employment, with CPR certification required. Experience under supervision is preferred.

Qualifications

  • Experience under a qualified registered occupational therapy supervisor is preferred.

Job description

The Occupational Therapist Home Health works collaboratively with other members of the interdisciplinary team to ensure delivery of high quality occupational therapy services in accordance with the physician’s orders and with the agency’s programs and policies. Services are provided in the patient’s place of residence.

Responsibilities
  • Performs initial evaluation to identify functional deficits.
  • Establishes a Plan of Treatment in consultation with the physician, including short‑ and long‑term goals.
  • Guides the patient in the use of therapeutic, creative, and self‑care activities to improve or restore function and to apply function to practical use.
  • Plans an individualized program utilizing physical or mental activities, crafts, avocations, exercises, and daily self‑care activities to reach maximal level of function.
  • Instructs in the use of equipment that will help improve function in the home.
  • Observes, records, and reports the patient’s response to treatment and any changes in condition to the physician.
  • Communicates with the Rehab Program Manager, Patient Care Manager, Clinical Coordinator, and other staff to plan total patient and family care.
  • Confers with other disciplines as appropriate to plan rehabilitation for the patient.
  • Maintains records that reflect a family‑oriented approach.
  • Ensures documentation is timely, accurate, legible, and in compliance with agency policy and directives.
  • Documents goals and plans made with the patient and/or family.
  • Evaluates and documents the effectiveness and outcomes of care on an ongoing basis, making revisions as needed.
  • Plans for discharge as appropriate.
  • Instructs other health team personnel, when appropriate, in occupational therapy areas such as activities of daily living, working with patients, and involving family members.
Physical and Sensory Requirements
  • Must be able to sit for up to ninety minutes at a time.
  • Must be able to tolerate standing for thirty to forty‑five minutes consecutively.
  • Must be able to use hands and fingers to manipulate dials on machines.
  • Must be able to lift or move patients up to 300 lbs, and push or pull patients weighing up to 300 lbs.
  • Must frequently stoop, bend, squat, crouch, kneel, and reach above shoulder level.
  • Must have tactile sensation to feel muscle spasms, texture, and skin temperature.
  • Must have normal hearing for conversation.
  • Must have general vision, depth perception, and peripheral vision.
Driving and Transportation
  • Must have and maintain a valid PA or NJ driver’s license.
  • Must maintain a good driving record accepted by the network insurance carrier.
  • Must maintain motor vehicle insurance with the minimum requirements established by PA or NJ.
  • Must obey all motor vehicle regulations and laws while operating the vehicle.
  • Must perform routine vehicle maintenance.
Education
  • Bachelor’s Degree in Occupational Therapy required.
  • Master’s Degree preferred.
  • Occupational Therapist license in the state of employment required.
Training and Experience
  • Preferably one or two years of experience under a qualified registered occupational therapy supervisor.
Certification
  • CPR certification required.
