Optimal Care - Evergreen is seeking a Home Health Medical Social Worker in Mid-M Michigan to support patients at home. The role involves psychosocial assessments, counseling, and coordinating community resources to enable recovery and independence.

You will collaborate with nurses, therapists, physicians, and caregivers to craft personalized care plans that address social determinants of health and family dynamics. Travel daily within the service area is required.

Qualifications

• MSW degree from a CSWE-accredited program.
• Current State License as MSW/LMSW/LCSW.
• Minimum 1 year of licensed MSW experience.
• Reliable transportation with valid auto insurance.
• Willingness to travel daily for home visits within service area.

Responsibilities
