Job summary

AccentCare Inc. is seeking a Licensed Masters Social Worker to join our home health team in California.

This PRN, on-site role involves traveling to multiple locations within the coverage area to assess patients, develop and revise goals and care plans, and coordinate services for patients and families. The ideal candidate holds an MSW, is CPR certified, and has at least one year of healthcare social work experience.

Qualifications

• Master’s Degree in Social Work (MSW) required.
• One year of social work experience in a healthcare setting.
• Must possess and maintain CPR certification while employed in a clinical role.

Responsibilities

• Provide patient assessments and develop, evaluate, and revise goals and care plans.
• Deliver home health social work services to patients and families.
• Travel to assigned locations to provide care and coordinate services.
