Extended Home Care is seeking a qualified Registered Nurse (RN) to join our Home Care team in providing in-home patient care across communities. The RN plans, coordinates, and assesses care, emphasizing education and patient-centered services delivered in patients' homes.

Applicants must hold an active RN license in the state, current CPR certification, and typically at least two years of public health or home care experience. Strong self-direction and communication skills are essential.

Qualifications

• Current RN license in state of practice.
• Current CPR certification.
• Minimum two years’ experience in public health or home care nursing preferred.
• Must be a licensed driver with reliable transportation.

Responsibilities

• Complete initial patient and family assessment to determine home care needs.
• Perform comprehensive physical assessments and health histories.
• Regularly re-evaluate nursing needs and adjust plans of care.
• Administer medications and treatments per physician orders.
