Job summary

Sevita’s Massachusetts Mentor program seeks a dedicated Foster Care Case Manager to coordinate services, case management, behavior plans, and treatments for youth served in our community-based programs.

You will oversee all services and ensure quality and regulatory compliance while building strong relationships with families and external case managers. This is a full-time role offering a comprehensive benefits package and opportunities for growth.

Qualifications

• Bachelor’s degree in human services or related field.
• One year of work-related experience with individuals with intellectual and developmental disabilities, brain injury, youth at-risk, etc. (preferred).
• Current driver’s license, car registration, and auto insurance.
• Licensure and training as required by state (e.g., CPR, First Aid, Behavioral Intervention Techniques).
• A reliable, compassionate attitude and a commitment to quality in everything you do.

Responsibilities

• Coordinate the development, review, and implementation of services, case management, behavior intervention plans, and treatments provided to individuals served.
• Oversee all services provided to individuals and their families, monitor quality of care, ensure contractual compliance, and monitor regulatory program compliance.
• Assist in the development of quality services and engaging activities that meet the individual served needs.
• Build and maintain relationships with families and external case managers.
