Job summary

Sevita’s Pennsylvania Mentor is hiring a Case Manager Foster Care to support youth in foster care, transitioning back home, or exploring independent living options. You will coordinate services, ensure quality and regulatory compliance, and collaborate with families and external case managers to tailor plans that meet individual needs.

The role emphasizes care coordination, relationship-building, and delivering compassionate, quality services within a mission-driven team.

Qualifications

• Bachelor’s degree in social work, criminal justice, or education is required.
• At least 2 years of case management experience, including 1 year with at-risk youth.
• Valid driver’s license and vehicle insurance are needed; state-specific licensure/training may apply.

Responsibilities

• Coordinate development, review, and implementation of services and case management.
• Oversee services to individuals and families, monitor quality and compliance with contracts.
• Contribute to development of quality services and engaging activities for individuals served.
• Build and maintain relationships with families and external case managers.
