Wheeler Clinic is seeking a Case Manager for the Extended Day Treatment program to provide case management for children and families in Plainville, CT. The role involves coordinating referrals, counseling, and treatment planning within a collaborative team environment.

The position requires a BA/BS degree and CT driver’s license with the A endorsement within 90 days. Full-time schedule with comprehensive benefits and growth opportunities are offered.

Qualifications

• BA/BS degree in related field is required.

Responsibilities

• Coordinates referrals and maintains linkages with community sources.
• Co-facilitates group and/or individual counseling with supervision and coordinates treatment phases.
• Maintains a structured clinical milieu using a team approach with evidence-based practices.
• Develops case management plans for each client and family to increase treatment completion.
• Completes documentation for client medical records per timelines and regulatory requirements.
• Attends team meetings to review treatment plans and coordinate care.
• May transport clients to appointments or community linkages as needed.
• Provides services in office and community settings outside of the office.
