Job summary

Lutheran Family And Childrens Services Of Missouri is seeking a compassionate care coordinator to support families and children across Missouri. The role focuses on coordinating care, providing case management, and connecting clients with community resources to promote safety, stability, and well-being.

The ideal candidate holds a Bachelor's or Master’s degree in a human behavior field, is a licensed driver, and has a passion for making a difference.

Qualifications

Bachelor's or Master's degree in a human-behavior field (Social Work, Counseling, Psychology, Criminal Justice, etc).
Candidate must be a licensed driver with access to dependable personal transportation and auto insurance.
Demonstrated passion for helping families and making a difference in the community.
Preferred 2 years of experience with Child Welfare, Adoption, Foster Care, or related community resources.

Responsibilities

Coordinate and provide care to clients that are safe, timely, effective, efficient, equitable, and client-centered.
Provide case management, parenting education, community resource referral, parent-child activities, and child development education.
Create a service plan in conjunction with the client to develop goals and objectives that align with client wishes.
Counsel, advocate, and teach clients the skills identified in their service plan face to face in home or virtually.
Respond to client crises within program parameters in a timely manner according to program protocols.
