Job summary

The Jane Pauley Community Health Center, Inc. is seeking a Family Nurse Practitioner (FNP) to join our medical team in Indiana.

We are a Federally Qualified Health Center delivering integrated, whole-person care to diverse communities across the state. The ideal candidate will provide full-scope primary care across the lifespan, collaborate with physicians and care teams, and contribute to quality improvement while upholding HIPAA and regulatory standards.

Qualifications

• Master’s or Doctoral degree in Nursing from an accredited program.
• Board certification as a Family Nurse Practitioner (FNP).
• Prescriptive authority and DEA licensure (or ability to obtain).
• Strong clinical assessment, diagnostic, and decision-making skills.
• Excellent communication and collaboration skills.

Responsibilities

• Conduct patient assessments, physical exams, and diagnostic evaluations.
• Diagnose and manage acute and chronic conditions across the lifespan.
• Develop and implement individualized treatment plans.
• Prescribe medications and perform procedures within scope of practice.
• Provide preventive care, screenings, and health maintenance services.
• Educate patients and families on disease management, prevention, and wellness.
• Collaborate with care teams to ensure coordinated, patient-centered care.
• Support transitions of care and appropriate referrals within and outside the network.
• Maintain accurate and timely documentation within the EHR and ensure HIPAA compliance.
