Job summary

Jane Pauley Community Health Center seeks a Family Nurse Practitioner to deliver comprehensive primary care in Indiana’s FQHC setting. You’ll focus on prevention, diagnosis, and management of conditions across the lifespan, collaborating with physicians and care teams to provide patient-centered services.

You’ll prescribe medications, order tests, educate patients, and participate in population health and quality improvement initiatives.

Qualifications

• Master’s or Doctoral degree in Nursing from an accredited program.
• Current, unrestricted Indiana Nurse Practitioner license.
• Board certification as a Family Nurse Practitioner (FNP).
• Prescriptive authority and DEA licensure (or ability to obtain).
• Strong clinical assessment, diagnostic, and decision-making skills.
• Excellent communication and collaboration skills.

Responsibilities

• Conduct patient assessments, physical exams, and diagnostic evaluations.
• Diagnose and manage acute and chronic conditions across the lifespan.
• Develop and implement individualized treatment plans.
• Prescribe medications and perform procedures within scope of practice.
• Provide preventive care, screenings, and health maintenance services.
• Educate patients and families on disease management, prevention, and wellness.
• Collaborate with care teams to ensure coordinated, patient-centered care.
• Support transitions of care and appropriate referrals within and outside the network.
• Participate in quality improvement initiatives and care gap closure efforts.
