Job summary

You will follow care plans, document progress, assist with daily living tasks, transport clients to appointments, and collaborate with families and teammates.

Qualifications

• High school diploma or GED required.
• Ability to lift 50 lbs and assist mobility needs.
• Strong communication and basic computer skills.

Responsibilities

• Empower daily living by assisting with cooking, cleaning, hygiene, and health maintenance.
• Foster independence through social skills and community involvement.
• Follow care plans and document progress accurately.
• Provide transportation to appointments and activities.
• Collaborate with teammates and families to ensure quality support.
