Job summary

Claratel Behavioral Health is seeking a Direct Service Professional to support individuals with intellectual and developmental disabilities across home, work, and community settings in Georgia. The role focuses on building life skills, promoting independence, and ensuring safety and well-being.

You will assist with daily living activities, follow care plans, and document progress while collaborating with families and teammates.

Qualifications

• High school diploma or GED required.
• Valid Georgia Driver’s License with acceptable MVR.
• Ability to lift 50 lbs unassisted and assist mobility needs.
• Regular bending, squatting, standing, and moving throughout shift.
• Strong communication, basic computer literacy, and conflict management skills.

Responsibilities

• Provide support across home, work, and community settings to help individuals build life skills and achieve goals.
• Assist with activities like cooking, cleaning, personal hygiene, health maintenance, and medication self-administration.
• Teach social skills and independence; implement ISP/BSP and strategies as instructed.
• Follow care plans, medical instructions and dietary needs; report risks and safeguard individuals.
• Be a mentor and role model; coach individuals through challenges and support meaningful relationships.
• Prepare documentation: record progress, behavior tracking, incident reports, and logs on time.
• Provide transportation to appointments and activities; ensure safety during transit.
• Collaborate with teammates and families; pursue ongoing professional development.
• Innovate by contributing ideas to improve service quality and operations.
