Job summary

Intention Healthcare, an Essen Health Care Company, is seeking a Complex Case Manager (Social Services) to support high-need patients through coordinated care and social support services. The CCM collaborates with primary care and the care team to identify needs and connect patients with community resources.

The role involves home visits, patient education, and facilitating transitions of care from hospital or skilled facilities back to home, along with addressing adherence and care coordination

Qualifications

• Bachelor's degree in Social Work or similar required.
• 2+ years of experience in health care or health-related social work.
• Strong customer-service orientation and verbal communication skills.
• Ability to work independently and exercise judgment in interactions with providers, patients, and families/caregivers.
• Good understanding of health care and health insurance concepts.

Responsibilities

• Develop rapport with patients and family, conducting frequent check-ins and winning the patient’s trust to be their first call for health-related matters.
• Occasionally visit patients at home to conduct safety assessments, patient education, and address social needs.
• Perform root-cause analysis of patient’s hospitalizations and identify where gaps occurred and how to intervene in the future.
• Educate about, refer to, and help patients apply to community resources, city and state programs.
• Assist with and facilitate the transition of care from inpatient settings to home.
• Address medication adherence and other treatment adherence.
• Communicate changes in patient’s status timely with the care team.
• Work with Administrative staff to provide feedback to improve day-to-day processes.
