Qualifications

• One of: sterile IV admixture experience, MTM, ASHP residency or APPE, or equivalent.
• Compliance with state/federal rules and safety standards.

Responsibilities

• Determines suitability of patients for home care and collects necessary data.
• Designs, monitors, and evaluates patient-specific pharmacotherapy plans.
• Provides medication-use education to patients, caregivers, and staff.
• Prepares and dispenses medications following policies and DEA regulations.
• Ensures continuity of pharmaceutical care to home and other settings.
