Copper Country Community Mental Health is seeking a full-time Client Services Manager/Supports Coordinator to deliver services in Ontonagon County, Michigan, with travel to surrounding counties as needed. The role focuses on individuals with serious mental illness and/or intellectual/developmental disabilities, including those with co-occurring substance use disorders.

Offering a competitive starting salary of $48,695–$63,886 annually (based on licensure and experience) plus a $5,000 sign-on

Formación

• Bachelor’s or Master’s degree in Social Work, Psychology, Counseling preferred; other Human Service or Nursing degrees will be considered.
• Current Michigan licensure or eligibility to obtain licensure in Michigan.

Responsabilidades

• Provide Supportive Services to individuals with serious mental illness and/or developmental disabilities.
• Conduct Assessments to evaluate needs and conditions.
• Create Individual Service Plans using a person-centered approach.
• Monitor and Evaluate progress and effectiveness of service plans.
• Maintain Confidential Records with privacy and security.

Educación
