Copper Country Community Mental Health is hiring a full-time Client Services Manager/Supports Coordinator to provide services in Ontonagon County, Michigan, with travel to other counties as needed. This role works with people with serious mental illness and/or intellectual/developmental disabilities, some with co-occurring substance use disorders.
The position offers a salary based on licensure and experience, plus a $5,000 sign-on bonus, and comprehensive benefits.
Formación
- Bachelor's or Master's in Social Work, Psychology, Counseling or related Human Service field.
- Current Michigan licensure or eligibility to obtain licensure in Michigan.
- Experience with individuals who have intellectual/developmental disabilities and mental illness.
Responsabilidades
- Provide Supportive Services to individuals with serious mental illness or developmental disabilities.
- Conduct Assessments to evaluate needs and conditions.
- Create Individual Service Plans using a person-centered approach.
- Monitor and Evaluate progress to ensure effectiveness of plans.
- Maintain Confidential Records in line with privacy standards.
