Responsibilities

• Coordinate planning services by interviewing client, assessing needs and goals, and linking to services
• Educate client on program requirements, options, interpretations, and determinations
• Monitor service delivery through follow-up interviews with client or providers
• Provide emergency/crisis intervention by referring to appropriate programs
• Maintain client records and complete required paperwork accurately
• Ensure compliance with federal, state and local laws by reviewing legislation and informing management
• Maintain confidentiality and professional boundaries with clients
• Foster cooperation across departments to maintain workflow
