Job summary

Qualifications

• Bachelor's degree or four years of direct human services experience.
• Valid Driver’s License.
• Background check clearance.
• Care coordination training completed after hire by ND.

Responsibilities

• Direct Engagement: conduct weekly meetings with program participants to monitor progress.
• Care Planning: assess needs and develop participant-centered care plans.
• Inter-Agency Collaboration: partner with local agencies to support recovery, housing, employment and reduce recidivism.
• Documentation: maintain timely, accurate client interactions and compliance.
