Qualifications

• Bachelor’s degree in Social Work, Psychology, Sociology, Nursing, or related human services field.
• 2 years of professional experience in case management, child welfare, adoption services, or related field; master’s degree may substitute for up to one year of experience.
• Proficiency in MS Office (Word, PowerPoint, Excel) and Teams.

Responsibilities

• Provide emotional and practical support to expectant and birth parents with a holistic, client-centered approach.
• Collect background information, assess needs, and develop customized case management plans.
• Facilitate communication between birth parents and prospective adoptive families.
• Assist in matching, placement, and post-placement supervision of children in adoptive homes.
• Complete and document home studies and post-placement reports per agency/state/federal requirements.
• Identify and utilize agency, state, federal, and community resources to support families.
• Coordinate local resources across diverse geographic locations.
• Maintain accurate documentation in case files and systems.
• Close adoption files and ensure records sealing per guidelines.
• Flexible problem-solver; may work evenings/weekends as needed.
• Perform other duties to support organizational objectives.
