Qualifications

• Bachelor’s degree in a human services field is required or equivalent.
• 2 years of case management or related experience with clients and families.
• Strong communication and organizational skills.

Responsibilities

• Provide emotional and practical support to expectant and birth parents, with a holistic, client-centered approach focused on both the parent and child’s well-being.
• Collect comprehensive background information, assess individual needs, and develop customized case management plans.
• Facilitate communication between birth parents and adoptive families, ensuring all parties feel informed and supported.
• Assist in matching, placement, and post-placement supervision of children in adoptive homes, with sensitivity to emotional complexities.
• Complete and document home studies and post-placement reports per requirements.
• Identify and utilize agency, state, federal, and community resources to support families.
• Locate and coordinate local resources for clients, including across diverse locations.
• Maintain accurate documentation in client files and case management systems.
• Close adoption files and ensure records are sealed per guidelines.
• Be flexible and adapt to changing needs, including evenings and weekends when necessary.
• Perform other duties and special projects to support objectives.
