Responsibilities

• Intervenes and evaluates consumers, establishes preliminary diagnosis and develops treatment plans with goals and timelines.
• Provides crisis intervention to stabilize conditions and refers to other services as needed.
• Carries a caseload of Community Support Team clients and completes monthly direct service hours.
• Facilitates case management and therapeutic interventions in community settings, promoting independent living skills.
