Job summary

Cherokee Indian Hospital seeks a Qualified Mental Health Professional to provide case management and linkage to medical, behavioral, social, and other services for individuals with mental health and substance use disorders.

Responsibilities include assessments, person-centered planning, referral/linkage, and follow-up, with documentation in CIHA systems and collaboration with the treatment team. A valid driver’s license and relevant licensure are required.

Qualifications

• Qualified Professional means, within the MH/DD/SAS system of care, an individual with licensure/certification in a human service profession and four years of full-time behavioral health experience.
• Bachelor's degree in any field and four years of post-bachelor's experience in behavioral health or related areas.
• Bachelor's degree in any human service field with two years post-bachelor's experience in behavioral health and licensure/certification; or
• Master's degree in a human service field and one year of post-graduate experience in behavioral health or substance abuse.

Responsibilities

• Provide QMPH/case management functions including assessment, person-centered planning, referral/linkage, and follow-up.
• Develop and implement discharge plans and transfers to other facilities as appropriate.
• Collaborate with medical staff and other health professionals to facilitate patient care, discharge, and follow-up.
• Maintain timely records of treatment and events in the medical record per CIHA policies and procedures.
• Provide consultation to patients and families regarding limitations, finances, and ongoing treatment.
