Qualifications

  • Requires relevant professional license or permit in a human service field.
  • Four years of full-time behavioral health/developmental disabilities/substance abuse experience with certain licenses.
  • Bachelor’s degree + four years post-bachelor’s in related fields or licensure.
  • Master’s degree with required experience in behavioral health or licensure.
  • Must possess a valid driver’s license.
  • Specific experience with Native Americans preferred.

Responsibilities

  • Perform case management assessment and person-centered planning.
  • Provide referrals, linkage, monitoring, and follow-up.
  • Document information across medical, psychosocial, financial, and cultural areas.
  • Develop and implement person-centered plans to achieve outcomes.
  • Coordinate with healthcare teams for discharge and follow-up.
  • Maintain HIPAA and privacy compliance and accurate records.
