Job summary

Community Options is seeking a Care Coordinator in North Dakota to manage client caseloads within our behavioral health programs. You will conduct intakes, assess needs, and develop care plans to promote client independence by removing barriers.

Build relationships, coordinate with providers, and refer to internal/external resources to support outcomes. You will facilitate client team meetings and ensure follow-up to achieve goals, with travel as needed and a focus on compassionate,

Qualifications

• Associates degree in a related field or 1 year of direct client services experience required.
• Ability to obtain valid driver's license and travel as needed.
• Must complete care coordination training post-hire and pass background checks.
