Job summary

Hamilton County Developmental Disabilities Services in Cincinnati, OH is seeking an Assessment Specialist to perform assessments, gather information from individuals, families, and teams, and document findings accurately. The role includes coordinating with internal departments and external providers, making referrals based on identified needs, and sharing information with the ongoing support team.

After assessments, individuals are transitioned to a designated team for ongoing support.

Qualifications

  • Bachelor's degree in a related field is required.
  • One year of full‑time paid work experience in programs or services for individuals with developmental disabilities.
  • 1.5 years of experience conducting relevant assessments.
  • Ability to travel within Hamilton County as needed.

Responsibilities

  • Administer, score, and submit InterRAI and Ohio Assessment tools in required state and county systems within established timelines.
  • Conduct in-person and phone assessments for individuals and families with developmental disabilities to determine eligibility for County Board services and nursing home placement.
  • Evaluate individuals using state-approved instruments, applying independent judgment to make recommendations for services and Medicaid waiver funding.
  • Collect and analyze records; interview individuals served, families, and support staff to inform assessment findings.
  • Ensure compliance with state regulations, Medicaid waiver requirements, and confidentiality standards.
  • Communicate assessment findings to internal teams, provider agencies, and other professionals to support service planning.
  • Coordinate waiver waiting list processes and monitor individuals whose needs are met through alternative resources.
  • Research and connect individuals and families with appropriate community resources.
  • Maintain accurate, timely documentation and progress tracking in required databases.
  • Travel to worksites and community locations as needed.
  • Represent the agency professionally to build and maintain community partnerships.
  • Perform additional duties as assigned.

Education

Bachelor's degree in special education, early childhood, social work, or related field
