Qualifications

• Graduation from Accredited APRN Certification Program.
• Board Certified.

Responsibilities

• Provide comprehensive primary care focused on health promotion, illness prevention, and health restoration for patients across the lifespan.
• Conduct thorough patient histories and physical examinations, assess health concerns, and develop appropriate medical and/or nursing diagnoses.
• Order, perform, and interpret appropriate diagnostic evaluations and coordinate consultations with specialists and other healthcare providers as needed.
• Develop individualized treatment plans including prescribing medications, interventions, and follow-up care.
• Monitor patient progress and coordinate ongoing care for timely, appropriate services.
• Provide education and guidance on health maintenance and disease prevention to patients and families.
• Collaborate with physicians, nurses, and care team members to support patient-centered care and daily team huddles.
• Serve as a clinical resource by providing consultation and support on health maintenance and referrals.
• Participate in quality assurance, patient care, and program review meetings; complete continuing education to maintain APRN licensure.
• Mentor and precept Physician Assistants, Nursing, and APRN students.
