Job summary

Evara Health in Pinellas Park, FL, is seeking an Advanced Practice Registered Nurse to deliver comprehensive primary care within a Patient-Centered Medical Home model. You will work with a multidisciplinary team to assess, diagnose, treat, and refer patients across the lifespan.

Qualifications

• Graduation from an accredited APRN program and board certification are required.
• Completion of an APRN certification process and maintenance of licensure.

Responsibilities

• Provide comprehensive primary care focused on health promotion and disease prevention.
• Perform histories and exams, develop diagnoses, and coordinate care.
• Order and interpret diagnostics and coordinate with specialists as needed.
• Develop and adjust personalized treatment plans and follow-up care.
• Monitor progress and ensure timely, appropriate services.
• Educate patients and families on health maintenance and healthy lifestyles.
• Collaborate with care team and participate in daily huddles.
• Serve as a clinical resource for staff and patients.
• Participate in QA, program reviews, and continuing education for licensure.

About This Role:

The Advanced Practice Provider provides comprehensive care for common acute and chronic health conditions while collaborating with a multidisciplinary care team. This role supports the Patient-Centered Medical Home model through direct patient care, coordination, consultation, education, and referrals.

What You'll Do:

• Provide comprehensive primary care focused on health promotion, illness prevention, and health restoration for patients across the lifespan.
• Conduct thorough patient histories and physical examinations, assess health concerns, and develop appropriate medical and/or nursing diagnoses.
• Order, perform, and interpret appropriate diagnostic evaluations, including laboratory and radiographic testing, and coordinate consultations with specialists and other healthcare providers as needed.
• Develop individualized treatment plans based on each patient’s needs, including prescribing medications, providing appropriate interventions, and establishing follow-up care.
• Monitor patient progress and coordinate ongoing care to ensure patients receive timely, appropriate, and comprehensive services.
• Provide education and guidance to patients and families on health maintenance, disease prevention, healthy lifestyle choices, and management of acute and chronic conditions.
• Collaborate closely with physicians, nurses, and other members of the care team to support coordinated, patient-centered care and actively participate in daily team huddles.
• Serve as a clinical resource by providing consultation and support to patients, families, physicians, care team members, and nursing staff on health maintenance, referrals, and patient care needs.
• Participate in quality assurance, patient care, and program review meetings, as designated by the Medical Director, and complete continuing education required to maintain APRN licensure and clinical competency.
• Mentor and serve as a preceptor for Physician Assistant, Nursing, and APRN students while supporting the development of future healthcare professionals.
