Job summary

Qualifications

• OT certification required (NBCOT)
• Illinois OT license required
• BLS certification required
• Master's degree in OT or higher required if graduated after 2007
• Bachelor of Science in OT if graduate before 2007 required

Responsibilities

• Administers occupational therapy evaluations and develops collaborative treatment plans with patients.
• Restores function and prevents disability through ongoing assessment and interventions.
• Provides direct patient care for majority of hours worked.
• Performs evaluations and re-evaluations as part of standard care.
• Designs and implements treatment plans and discharge/home care planning.
• Completes medical documentation in a timely, accurate manner per policies.
