Job summary

August Healthcare is seeking a licensed Physical Therapist (PT) to join our long-term care team and deliver high-quality, patient-centered therapy to residents. You will help improve mobility, function, independence, and overall quality of life while collaborating with an interdisciplinary care team.

The role offers full-time, part-time, and PRN options, with a strong emphasis on documentation, safety, and resident outcomes.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current and active state Physical Therapist license.
• Long-term care or skilled nursing experience preferred.
• Strong clinical documentation and communication skills.
• Ability to work effectively as part of a multidisciplinary team.
• Patient-focused and committed to quality care.

Responsibilities

• Evaluate residents and develop individualized treatment plans based on clinical needs.
• Provide skilled physical therapy services per care plans.
• Maintain accurate documentation: evaluations, notes, progress reports, discharge summaries.
• Collaborate with nursing, therapy, and interdisciplinary team members.
• Participate in care planning, utilization review, and meetings.
• Educate residents, families, and staff on goals, plans, safety, and mobility.
• Maintain productivity and compliance with standards.
• Support regulatory compliance and survey readiness.
• Promote safety, dignity, independence and quality of life.
