Job summary

Leewood Healthcare Center in Annandale, VA is seeking a licensed Physical Therapist (PT) for full-time, part-time, or PRN roles to provide skilled therapy to residents under physician orders.

You will evaluate, treat, document, and collaborate with an interdisciplinary team to promote mobility, safety, and independence; a $10,000 sign-on bonus is offered for full-time hires and H-1B sponsorship is available.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current and active state license as a Physical Therapist.
• Long-term care or skilled nursing experience preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans based on clinical needs and physician orders.
• Provide skilled physical therapy services in accordance with established care plans.
• Monitor resident progress and modify treatment approaches as clinically appropriate.
• Complete accurate and timely documentation, including evaluations, treatment notes, progress reports, and discharge summaries.
• Collaborate with nursing, therapy, and interdisciplinary team members to achieve positive resident outcomes.
• Educate residents, families, and staff regarding therapy goals, treatment plans, safety, and functional mobility.
• Maintain established productivity, documentation, and quality standards.
