Qualifications

Responsibilities

• Coordinates on-site, confidential child-facing services and case management activities.
• Completes trauma-focused initial intake assessment within 24 hours of arrival and orients youth to the agency and foster home.
• Coordinates contact of youth sponsors immediately upon arrival.
• Ensures youth receive mandatory services (mental health, medical, recreational, educational) in a timely manner.
• Maintains accurate youth records and complies with documentation standards.
• Attends treatment team meetings and collaborates with stakeholders to resolve complex case issues.
• Reviews home visit reports and sponsor assessments to support best interests of the child.
• Provides training and supervision for case managers and ensures compliance with program requirements.
