Job summary

Jobtailor is seeking a dedicated case manager to assess client needs across housing, food, childcare, transportation, income, healthcare, technology, and financial assets. You will co-create individualized service plans with measurable actions and provide follow-up to ensure progress.

You will make warm referrals to community resources and support Catalyst for Economic Mobility participants, while maintaining up-to-date knowledge of local resources, eligibility, waitlists, and geography.

Qualifications

• Client needs assessment and service planning skills are essential.
• Ability to navigate community resources and coordinate referrals.
• Experience with motivational interviewing or strength-based coaching is desirable.

Responsibilities

• Conduct intake sessions to assess client needs across housing, food, childcare, transportation, income, healthcare, technology, and financial assets.
• Co-create individualized service plans with measurable client-owned action items.
• Provide follow-up and intentional conversations to review goal achievement.
• Make warm referrals to community resources, financial coaching, workforce development, and other supports.
• Provide support, monitoring, and coordination for Catalyst for Economic Mobility participants.
• Maintain current knowledge of local community resources and verify eligibility, waitlists, and geographic availability.
• Develop and maintain relationships with community partners and referral agencies.
• Support clients in navigating applications and systems when barriers require assistance.
• Complete program data entry for outcome tracking, reporting, and funder compliance.
