Meridian Services seeks an Individualized Home Support (IHS) Specialist in the Minneapolis, MN area. Schedule is Monday–Friday 2:00 PM–9:00 PM, with $30.00 per hour and full-time benefits. You will support individuals in their homes to achieve personal goals through tailored activities and community engagement.

Meridian Services offers person-centered supports and cares to individuals with mental illness, intellectual disabilities, and physical disabilities across a diverse range of settings. These include Residential Services, In-Home Services, and Case Management Services. Today, we have 30 group homes, 150 In-Home person served, and over 100 Case Managers across the state of Minnesota. Job Title: IHS Specialist

Schedule: Monday-Friday 2:00PM - 9:00PM

Wage: $30.00 per hour including Full-Time company benefits (medical, dental, vision, and more)

Location: Minneapolis, MN and throughout the Twin Cities area

Job Summary: As an Individualized Home Support (IHS) Specialist, you will assist individuals in their personal residence or family home in working toward their personal goals. DSP staff support individuals with tasks such as meal preparation, community outings, shopping, cleaning, attending appointments, and other activities in their homes and communities.

Essential Job Duties:

• Work on tailored outcomes with each person served. Outcomes may include budgeting, behavioral management, community exploration, meal planning, organizational skills, coping skills, and/or physical fitness
• Be an active participant with individuals served when working on outcomes and when out in the community
• Provide guidance and instructional teaching to persons served while building life skills
• Provide supervision and transportation in the community for activities and appointments.

Required Qualifications:

• Staff must be at least 18 years of age to support Adult Programs
• Proficient in spoken and written English language
• Have a vehicle to use for work related purposes with current car insurance
• Have an acceptable driving record as determined by Meridian Services policy
• Have a Wi-Fi-enabled device for work related purposes
• Successful clearance of a Department of Human Services (DHS) Background Study

Preferred Experience:

• At least 1 year supporting individuals with autism, intellectual disabilities, and/or mental health diagnosis
