We seek an experienced Case Manager for our IHBT team, providing therapeutic services in clients' homes and communities. Your role involves developing treatment plans, implementing strategies, and collaborating with a team to promote client stability and symptom management.

Primary Duties
  • Develop individualized treatment plans with clients and families.
  • Set measurable treatment goals aligned with client needs.
  • Provide ongoing support, guidance, and advocacy.
  • Monitor progress and adjust plans as needed.
  • Participate in crisis interventions and coordinate with emergency services.
  • Facilitate meetings and provide psychoeducation.
  • Assist clients in accessing community resources.
  • Collaborate with other professionals involved in care.
  • Complete documentation and reports promptly.
  • Attend meetings and training to maintain compliance and fidelity standards.
Qualifications

At least one year of paid experience working with youth with significant mental health concerns and their families is required.
