IHBT Case Manager Position

Camelot seeks an Experienced Case Manager for an Intensive Home-Based Treatment team. You will collaborate with a therapist to develop treatment plans, implement strategies, and coordinate care to promote stability and symptom management in youth with mental health needs.

Primary Duties:

  • Develop individualized treatment plans with clients and families.
  • Set measurable, achievable goals aligned with client needs.
  • Provide ongoing support, guidance, and advocacy.
  • Monitor progress and adjust plans as needed.
  • Participate in on-call crisis intervention, both telephonically and in person.
  • Collaborate with emergency and community response teams.
  • Facilitate meetings on coping skills and emotional regulation.
  • Provide psychoeducation on mental health and medication adherence.
  • Assist families in accessing community resources.
  • Work with other professionals involved in care.
  • Complete required documentation and reports promptly.
  • Attend meetings and training to maintain compliance and fidelity to IHBT standards.
Qualifications

Education, experience, and skills:

Requires at least one year of paid experience working with youth with significant mental health concerns and their families.
